Accreditation Family Evaluation of Program

We are providing an opportunity for families to participate in the self-assessment and program improvement process. Results of this assessment and program improvement goals will be published in the newsletter.

For each statement, select "Yes" or "No" or "DK" for "Don't Know". If the statement does not apply to your child's program, select "NA" for "not applicable."
